• Initiating Advance Care Planning for patients and completing comprehensive assessments and care plans
• Coordinating discharges, placements, and continuation of coverage for patients
• Building and maintaining professional relationships with referral sources, patients, and families
• Documenting patient interactions and maintaining accurate records
• Meeting daily productivity goals and demonstrating excellent time management and organization skills

To succeed in this role, you will need:

• Excellent customer service skills and ability to communicate effectively with patients, families, and healthcare teams
• Strong computer technology skills, including EMR experience and ability to navigate multiple software applications
• Time management, organization, and placement skills
• Ability to work independently and as part of a team in a remote setting
