Position Overview – Social Worker, Home Health (Per Diem)

arenaflex is urgently seeking a skilled and empathetic Social Worker to join our Home Health team on a per‑diem basis in Chattaroy, WA. This role is pivotal in the interdisciplinary care team, providing comprehensive psychosocial assessments, resource coordination, and emotional support to patients navigating complex medical conditions. The successful candidate will work closely with physicians, nurses, therapists, and family members to develop and implement individualized care plans that address both medical and social determinants of health.

Key Responsibilities

  • Comprehensive Assessment: Conduct thorough evaluations of patients’ social, emotional, and environmental circumstances, identifying barriers to recovery such as housing instability, financial strain, or limited community resources.
  • Care Planning & Coordination: Collaborate with the interdisciplinary team to design, implement, and periodically reassess personalized care plans that integrate medical treatment with psychosocial support.
  • Resource Mobilization: Locate, negotiate, and secure community resources—including housing assistance, transportation services, counseling, legal aid, and financial programs—to address identified patient needs.
  • Family Education & Support: Provide education to patients and families on coping strategies, stress management, and navigating the healthcare system, ensuring they feel confident and informed.
  • Discharge Planning: Partner with physicians and case managers to develop safe, sustainable discharge plans that minimize readmission risk and promote continuity of care.
  • Documentation & Reporting: Maintain accurate, timely documentation of assessments, interventions, and outcomes in accordance with arenaflex policies, regulatory standards, and best practice guidelines.
  • Supervision & Mentorship: Oversee the performance of Social Work Assistants (BSW), providing guidance, training, and performance feedback to ensure high‑quality service delivery.
  • Interdisciplinary Collaboration: Act as a resource for other team members, offering insights on social determinants of health and facilitating problem‑solving discussions.
  • Continuous Quality Improvement: Participate in case conferences, quality audits, and professional development activities to enhance service effectiveness and patient satisfaction.
  • Compliance & Ethics: Adhere to arenaflex’s policies, state and federal regulations, and professional ethical standards in all aspects of practice.

Essential Qualifications

  • Master’s or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
  • Current licensure as a Social Worker (LCSW, LMSW, or equivalent) in the state of Washington, or eligibility to obtain licensure.
  • Minimum of one year of professional experience in a health‑care setting, preferably home health, hospice, or acute care.
  • Valid driver’s license, reliable automobile, and auto insurance to travel to patient homes within the Chattaroy service area.
  • Current CPR certification (or willingness to obtain within 30 days of hire).
  • Demonstrated ability to assess and address discharge planning needs, including familiarity with community resources such as housing, financial assistance, and legal services.
  • Excellent oral and written communication skills, with the ability to convey complex information in a clear, compassionate manner.
  • Proficiency in English (reading, writing, speaking) and basic computer skills for electronic health record documentation.
  • Strong organizational abilities, capable of managing multiple caseloads, prioritizing tasks, and thriving in fast‑paced environments.

  • Previous supervisory experience or mentorship of junior social work staff.
