Why Join Us · Immediate openings available · Flexible PRN schedule · Competitive pay · Supportive leadership and clinical team · Autonomy in the field · One-on-one patient care · Rewarding work with meaningful patient outcomes Responsibilities · Complete initial and ongoing physical therapy evaluations in patients’ homes · Develop and carry out individualized treatment plans based on physician orders and patient goals · Provide skilled interventions including gait training, strengthening, balance training, transfer training, mobility training, and fall prevention · Assess home safety and recommend equipment or modifications as needed · Educate patients, families, and caregivers on exercises, precautions, and safety strategies · Collaborate with physicians, nurses, and interdisciplinary team members to support continuity of care · Complete accurate and timely documentation in compliance with company and regulatory standards · Monitor patient progress and adjust treatment plans as appropriate Qualifications · Current, active Physical Therapist (PT) license in [State] · Graduate of an accredited Physical Therapy program · Minimum of 1 year of clinical experience preferred · Home Health experience preferred · Current CPR certification · Valid driver’s license, reliable transportation, and current auto insurance · Strong communication, clinical judgment, and time management skills · Ability to work independently and efficiently Ideal Candidate The ideal candidate is responsive, organized, and comfortable working independently in the home setting.
