Contact Center Triage Nurse – Part Time, MA CT

R38195 Commutable Remote Opportunity for a Nurse with telephonic triage experience who resides in either MA or CT Part Time Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to our Call Center. Baystate’s Call Center, located in Holyoke, MA, supports our Baystate Medical Practices, the largest multispecialty group in the region. We are seeking an experienced Telephonic Triage Nurse to join our Primary Care team! The Contact Call Center Triage Nurse, triages patients telephonically to facilitate patients' access to the appropriate level of care by assessing needs, offering a clinical option, referral to health care providers, services, and community resources. Promotes wellness and informed decisions by providing health information, home care advice, and prevention concepts. The nurse ensures timely access to appropriate care and seamless coordination of services throughout the continuum. Works collaboratively with physicians and other health team members and is available to patients and families for care coordination.

• Answers clinically related inbound calls. Maintains current knowledge base including triage skills, appropriate teaching principles,
• Initiates all forms of communication for each encounter using accurate medical history and documentation, including patient education as well as reviewing test/procedure preps.
• Resource to assigned scheduling team responding to clinical questions as relates to prescriptions, appointments, medical management, practice protocols and appropriate population management.
• Responsible to gather information on diagnostic tests, previous hospitalizations and ambulatory care, leads communication to obtain an accurate medical history, fully understands reason for referral, or post hospital care.

​ Experience:

• MA RN Licenses
• 5 years nursing experience with 1-year telephonic triage required.
• Exceptional communication and computer skills
• Proficient in the use of multiple computer applications including EMR, spreadsheet programs and electronic medical records.

Schedule & Location

• Ability to work remote after successful onsite training.
• Monday - Friday Part Time 24 hours
• Remote Opportunity- after successful onsite training
• Commutable Remote, must reside in MA or CT
