Responsibilities:

• Conduct compassionate virtual health assessments and consultations for individuals in sober living facilities via telehealth platforms, ensuring personalized care and attention.
• Perform thorough physical examinations and health screenings tailored to the specific healthcare needs of patients in recovery, emphasizing preventive care and overall wellness.
• Collaborate closely with interdisciplinary healthcare teams to create and implement individualized patient care plans, prioritizing preventive measures and health education.
• Prescribe medications judiciously in adherence to evidence-based guidelines and best practices in addiction medicine, aiming to support patients on their recovery journey.
• Utilize telehealth tools to monitor patient progress, promptly address concerns, and offer ongoing support, fostering a supportive and encouraging environment.
• Maintain accurate and updated electronic health records (EHR) while adhering to regulatory standards and organizational protocols.
• Foster seamless coordination of care for patients in sober living facilities by collaborating effectively with on-site healthcare providers and facility staff.
• Keep abreast of the latest advancements in addiction medicine and telehealth practices, integrating new knowledge into patient care strategies to continually enhance service delivery.

Qualifications:

• Current and valid Nurse Practitioner license in the state of Colorado.
• Certification as an Adult or Family Nurse Practitioner.
• MAT (Medication-Assisted Treatment) certification preferred.
• Prior experience in addiction medicine or a related field is advantageous.
• Exceptional communication and interpersonal skills, vital for engaging patients and collaborating within a virtual healthcare team effectively.
• Comfortable working independently and proficiently managing telehealth appointments.
• Demonstrated dedication to patient-centered care and a holistic approach to supporting individuals recovering from substance use disorders.

Schedule & Pay:

• Part-time commitment of 3-5 flexible hours per week, allowing for accommodating patient needs. Remote work from home.
• Competitive pay based on experience, ranging from $40 to $65 per hour.
• Employment Type: Per Diem
