As a Virtual Care Coordinator, you will play a crucial role in empowering individuals to take control of their health and wellbeing.

Key Responsibilities:

• Conduct virtual phone conversations to manage patients' healthcare needs and develop tailored care plans addressing physical, mental, and preventative health challenges.
• Utilize cutting-edge technology platforms and Electronic Health Records (EHRs) to document patient visits and ensure seamless care coordination.
• Coach patients on treatment plans, including nutrition and wellness strategies, to promote holistic health and wellness.
• Connect patients with essential resources and prepare them for medical appointments to ensure comprehensive care.

Requirements:

• Active Compact LPN License
• Minimum 2 years of clinical experience, with care coordination experience preferred
• Proficiency in Electronic Medical Records and Microsoft Office
• Excellent communication and problem-solving skills

• Full-time position with a flexible schedule (40 hours/week, Monday - Friday, 8 am - 6 pm in the patient's time zone)
