Job Summary:

The RN Care Manager will be responsible for telephonically assessing, planning, implementing, and coordinating all case management activities with members across the continuum of care. You will utilize advanced clinical judgment and critical thinking skills to effectively manage members through their illness, ensuring the appropriate level of care is being provided and preventing unnecessary hospitalizations. Your goal will be to achieve optimal clinical, financial, and quality of life outcomes for our members.

Key Responsibilities:

• Serve as primary care manager for members identified for Care Management program
• Conduct holistic assessments of current health status and develop personalized care plans
• Collaborate with multi-disciplinary teams to address physical, behavioral, and social needs
• Educate and assist members with advanced care planning and symptom management
• Monitor and evaluate effectiveness of care plans and make adjustments as needed

Requirements:

• Minimum 3 years of professional/clinical nursing experience, with preference for care coordination experience
• 2+ years of telephonic and/or field care management experience
• Unrestricted RN license in New York and willingness to obtain licensure in other states
• Strong organizational, communication, and problem-solving skills
• Proficiency with electronic health records (EHR) systems and excellent analytic skills
