The successful candidate will be responsible for overseeing the disease registry database, conducting pre-visit chart reviews, coordinating care across the continuum, and participating in quality improvement activities. Responsibilities

Requirements
- Graduate of an accredited school of practical nursing.

- 2 years of ambulatory care experience required.

- Current licensed practical nurse (LPN) license in state of practice required.

- Proficiency in using computers, software, and web-based applications.

- Effective verbal and written communication skills.

- Ability to present information clearly and professionally.

Nice-to-haves
