Job title: Clinical Social Worker LCSW in Palm Bay, FL at Health First Company: Health First Job description: Job RequirementsPOSITION SUMMARYThe Social Worker LCSW is fully engaged in providing quality/no harm, customer service andstewardship by utilizing advanced psychosocial skills to facilitate the coordination of patient carethrough assessment, planning, intervention and evaluation. The Social Worker LCSW usescurrent knowledge to effectively evaluate, promote and utilize hospital and community resourcesand services to achieve optimal outcomes, serving as a role model for coordination ofpsychosocial services for assigned patients in collaboration with the patient, family, physician,patient care team, and payers.PRIMARY ACCOUNTABILITIES1. Collaboratively determines desired discharge goals and communicates all discharge plansand options to patient/family, physicians and staff members.2. Provides counseling and researches placement alternatives for substance abuse, mentalhealth, homeless and indigent patients.3. Facilitates and participates in transfer of Baker Act and Marchman placements to mentalhealth facilities. Provides therapeutic interventions with patients and families as needed.4. Completes mandatory reporting processes.5. Facilitates referrals for post-discharge psychosocial and therapeutic needs.6. In a crisis situation where safety of the patient or others is in immediate jeopardy, theLCSW can Baker Act a patient.7. Mentors social work students from local universities during their internship in thedepartment.8. Provides clinical supervision for social workers in the department and those pursuing theirLCSW.9. Identifies system/process breakdowns and improvement opportunities, documenting forthe Performance Improvement Plan.10.Intervenes and recommends appropriate referrals to resolve system barriers to patientprogression, collaborating with other disciplines as directed.11.Communicates avoidable delays to the RN Case Manager for collaboration anddocumentation in MIDAS.12.Ensures compliance with Federal and state regulatory standards of care, identifying riskmanagement and quality issues, and intervening or referring as indicated.Work ExperienceMINIMUM QUALIFICATIONS

• Education: Master’s of Social Work

• Licensure: Current Florida LCSW license

• Certification: Current American Heart Association Basic Life Support Healthcare

Provider Completion Card upon hire and maintained

• Work Experience: One (1) year of previous social work experience

• Knowledge/Skills/Abilities:

o Basic computer skills including knowledge of MS Office – Outlook and Wordo Current knowledge of healthcare economic issues impacting acute careo Working knowledge of social work scope of practice and current issues impactinghealthcareo Understanding of payer/reimbursement practices and regulationso Ability to understand and comply with Federal and state regulatory standards ofcareo Ability to effectively handle multiple demands in a short timeframePREFERRED QUALIFICATIONS

• Certification: CCM or ACM Case Management Certification

• Work Experience: Case Management experience in Hospital setting

PHYSICAL REQUIREMENTS (Sedentary)

• Majority of time involves sitting or standing; occasional walking, bending, stooping

• Long periods of computer time or at workstation

• Light work that may include lifting or moving objects up to 20 pounds with or without

assistance.

• May be exposed to inside environments with varied temperatures, air quality, lighting

and/or low to moderate noise

• Communicating with others to exchange information.

• Visual acuity and hand-eye coordination to perform tasks

• Workspace may vary from open to confined; on site or remote

• May require travel to various facilities within and beyond county perimeter; may require
