Weekend Referral/Intake Specialist – Join Our Fast-Paced Home Health Team! This is a remote position. Ideally the candidate resides in the Tucson area but not required. Applicant MUST have home health intake experience! Are you a self-starter with a passion for patient care and strong attention to detail? We’re looking for a Weekend Referral/Intake Specialist to join our dynamic and growing home health agency. • Manage weekend referrals from start to finish—review, scrub, verify, and coordinate
• Ensure compliance with PDGM guidelines, insurance requirements, and intake policies
• Obtain initial service authorizations and validate coverage
• Communicate with referral sources and clinicians to set patients up for seamless weekend admissions
• Help drive our mission forward by supporting patients as they transition into home care

Schedule: 8–10 hours each weekend (flexible based on referral flow) Key Responsibilities:

• Process and manage weekend referrals in accordance with state, federal, and ACHC standards
• Ensure accurate and compliant documentation and coding
• Collaborate with referral partners and field staff to meet patient needs
• Maximize reimbursement through insurance verification and prior authorization
• Coordinate with multiple providers to initiate care plans
• Be a knowledgeable point of contact for resources within and outside our agency

What We’re Looking For:

• Minimum 6 months experience in a home health intake role
• Strong communication and coordination skills
• Ability to work independently and prioritize tasks
• Bilingual a plus
• Passion for service, efficiency, and quality care
