Full Time RN Admission Nurse ($7,500 Sign on Bonus)

We are committed to helping patients and their families always put life first. Responsibilities: Performs admission and recertification assessments as delegated by the Clinical Director or designee. Provides professional nursing care by utilizing all elements of nursing process. Initiates the plan of care and makes necessary revisions as patient status and needs change. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. The Admissions Nurse submits Local Coverage Determinations as defined by Medicare and a Comprehensive nursing assessment to physicians, and obtains initial certification orders if not already obtained. Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver. Qualifications and Requirements: Graduate of an accredited school of nursing. Current RN licensing, in good standing. Valid drivers license, and auto insurance Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
