Disability Case Manager

We’re seeking an experienced Disability Case Manager to join our team and help ensure our clients have access to the resources they need to live their best lives.The Disability Case Manager will be responsible for helping clients understand and access their benefits, develop and implement plans for independent living, and coordinate services with other providers. This role requires a compassionate, organized individual with excellent communication and problem-solving skills, as well as knowledge of disability laws and regulations.If you’re passionate about making a positive difference in the lives of individuals with disabilities, we’d love to hear from you. Responsibilities: Assist clients in understanding and accessing their benefits. Develop and implement plans for independent living. Coordinate services with other providers. Maintain up-to-date knowledge of disability laws and regulations. Provide compassionate and organized support to clients. Monitor and evaluate client progress. Create and maintain client records. Advocate for clients’ rights and best interests. Communicate effectively with clients, families, and other providers. Identify and resolve any issues or conflicts in a timely manner.
