We are seeking a compassionate and dedicated Medical Assistant ("MA") or nurse ("LPN") to join our team as a Care Coordinator. In this role, you will work directly with a provider to coordinate care for high-risk and complex geriatric patients. Your expertise will be essential in guiding patients through their plan of care, ensuring they receive the proper services and fostering stronger relationships between patients and their healthcare providers. Care Coordinators are often the primary point of contact for the patients they serve. This role is ideal for those looking to make a tangible impact on a patients life by working with healthcare professionals of all backgrounds and specialties. A day in a Care Coordinators life includes: Engaging with patients and caregivers by phone, Coordinating preventive and diagnostic services; Working directly with providers in prescribing medications and treatment; Providing exceptional communication with patients, families and caregivers; Scheduling; Coordinating specialist care.
