Employ motivational interviewing techniques to educate and engage patients while managing follow-up care. Maintain and regularly update the patient registry. Accurately document care plans and clinical notes. Actively contribute to interdisciplinary care team meetings to ensure continuity of care and address patient needs. Required Skills Strong communication and organizational skills. Ability to deliver patient-centered care. Critical thinking and meticulous attention to detail. Experience with motivational interviewing techniques. Qualifications Active and unencumbered Licensed Vocational Nurse (LVN) license. A minimum of 2 years of experience as an LVN. Previous remote work experience is required. Bilingual ability is preferred but not mandatory.

Compensation
And Benefits Salary: (65,000 - )85,000 annually. Paid Time Off: 3 weeks total (including 2 weeks PTO + 6-7 federal holidays). 401(k) retirement plan.
