Remote Primary Nurse - Chronic Care | WFH

Position Overview: As a Remote Primary Nurse, you will deliver compassionate, comprehensive, and evidence-based care to patients managing chronic conditions. You’ll collaborate with a multidisciplinary team, utilize cutting-edge telehealth technology, and empower patients to achieve better health outcomes. Key Responsibilities: Serve as the primary point of contact for chronic care patients, offering support, education, and personalized care plans. Conduct telephonic or video assessments to evaluate patient needs, progress, and adherence to treatment plans. Coordinate care with healthcare providers, specialists, and community resources. Monitor and adjust care plans based on patient outcomes, ensuring optimal management of chronic conditions. Provide education on disease management, medication adherence, and lifestyle modifications. Document all patient interactions and maintain compliance with clinical protocols and HIPAA regulations. Qualifications: Active and unrestricted Registered Nurse (RN) license. Minimum of 3 years of clinical experience, preferably in chronic care, case management, or telehealth. Strong communication and interpersonal skills, with a passion for patient advocacy. Familiarity with electronic medical records (EMR) systems. Ability to work independently in a remote setting while collaborating effectively with a team. Preferred Skills: Experience in telehealth or remote nursing roles. Certification in chronic care or case management (e.g., CCM, CMCN). Bilingual skills are a plus.
