Collaborate with the care team to design, implement, and update comprehensive care plans. Maintain clear communication regarding patient progress with the clinical manager and care team. Ensure accurate and timely documentation of individualized patient care records. Monitor and document changes in pain and symptom management, including outcomes. Provide education to families regarding patient conditions and care requirements. Refer patients to other hospice care specialists as necessary. Fulfill on-call responsibilities, delivering essential services to patients and families as assigned. Undertake additional duties as needed. Required Skills Valid Registered Nurse license. Minimum of one year of relevant nursing experience. Current CPR certification. Possession of a valid drivers license, reliable transportation, and liability insurance. Qualifications Active RN license. A minimum of one year of nursing experience.
