Nurse Practitioner - Hospitalist (Hospital at Home), Mercy South

Hospital at Home Opportunity in St. Louis, MO! Mercy Hospital South is currently seeking a Nurse Practitioner or Physician Assistant to join our new and exciting program in St. Louis. Missouri. As part of Mercy’s mission to transform the care experience, we are launching a new Mercy Hospital-at-Home (HaH) program, in partnership with Maribel Health. The role of the Advanced Practitioner will be in the hospital setting screening patients who qualify for this new, innovative program. Inpatient experience required. Mercy and Maribel – Hospital at Home Patients who have presented to the Emergency Department and have acute care needs that require hospitalization are thoroughly screened for admittance to the HaH Program. Once “admitted”, these patients are transported home to receive all the acute care services during the typical 3–5-day admission. This includes twice daily nursing visits, medical direction oversight, medication management, equipment, and all additional program support.
