The role also involves a significant amount of travel, with a minimum of 75% required to cover multiple counties in Indiana. Requirements • Unrestricted Registered Nurse License in the state of Indiana. , • 2 years of experience in in-home case/care management. , • Experience working with the adult population. , • Knowledge of community health and social service agencies. , • Ability to use electronic information applications/software programs, including electronic medical records. , • Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel. , • Exceptional communication and interpersonal skills. , • Ability to work with minimal supervision. , • Full-time availability with a flexible work schedule. Nice-to-haves • Bachelor of Science in Nursing (BSN). , • 3-5 years of in-home assessment and care coordination experience. , • Experience with health promotion, coaching, and wellness. , • Previous managed care experience. , • Certification in Case Management. , • Motivational Interviewing Certification.
