Pharmacist (Remote Consulting)
Summary
The Pharmacist position functions as an integral member of the hospice care team, working as the clinical pharmacy specialist in collaboration with hospice care physicians and other health care professionals to ensure that medical therapy aligns with the goals of care throughout the course of treatment. This role is for an afternoon/evening shift position. This position includes shifts starting 12-4pm EST and ending between 8p-12am EST including an alternating weekend rotation. Changes in schedule can be re-evaluated with seniority/company growth/performance over time. RESPONSIBILITIES: Recommends goal directed medication therapy to reduce symptom burden Answers consults from care givers for hospice medication management Serves as subject matter expert (SME) for managing hospice medications Provides hospice clinical staff with education and training Evaluates drug therapy and communicates/documents findings and recommendations Recommends medications, including initiation, continuation, discontinuation and altering therapy, based on established protocols Assures optimal, effective pharmaceutical care and collaborative practice for hospice patients Maintains a current knowledge of all professional and technical developments and requirements affecting the defined area of responsibility Prepares information for, and attends IDG (Interdisciplinary Group) meetings when requested Processes admission, transfer, and discharge information by adjusting patient data in computer Performs other related duties as assigned to support achievement of Pharmacy goals and objectives. REQUIREMENTS: RPh required, PharmD preferred Full, current and unrestricted license to practice pharmacy in the state of operation Citizen of The United States of America Must pass a designated and/or random drug screening Must pass a background/security review Proficient in written and spoken English Experience with various web browsers (Google Chrome / Firefox), Google Docs, Google Sheets or similar software Proven ability to manage multiple projects at a time while paying strict attention to detail Excellent listening skills Excellent verbal and written communication skills (typing) PREFERRED EXPERIENCE: Completion of a PGY-1 or PGY-2 Residency in Palliative Care or Pain Management or equivalent experience in Hospice/Palliative Care/Pain Management. Board certification in a relevant specialty certified by the Board of Pharmaceutical Specialties Multidisciplinary team leadership experience in a variety of settings Experience in collaborative development and launch of new pharmacy or patient care services, including development of policies, procedures, tools and metrics Hospice experience Apply Job!
