The Transplant Liaison serves as a support throughout each patient's transplant journey, starting from the referral stage up to the completion of the transplant. With a focus on patient centered care, the TL drives coordinating patient care, collaborates with transplant center stakeholders, and establishment of strong partnerships with healthcare providers, local transplant partners, and community resources. Key objectives include improving waitlist management, transplant rates, and equitable access to resources contributing to the overall improvement of patient care and outcomes. A vital link between communities and the healthcare system, TLs advocate for patients and their care partners, fostering a robust connection with the communities they serve. Their role extends to serving as intermediaries between health and social services and the transplant community, aiding in facilitating access and refining the transplant process to improve transplant access and outcomes within DaVita. Partners with patients, advocates, and multidisciplinary teams throughout the transplant process to decrease barriers (medical, financial, etc,) Uses care coordination drive waitlisting and transplant outcomes including but not limited to medical, financial, psychosocial, nutrition, and pharmaceutical to ensure and integrated and holistic approach in navigating the healthcare system to facilitated waitlisting and transplant -Educate patients and care partners on health promotion, transplant, and living donation to support waitlisting and transplantation -Serves as a liaison to DaVita clinics, transplant centers, health care system, and healthcare teams in communication of patient needs. -Assess, plan, advocate, and offers emotional support for the patient and their care partners in coordination with DaVita clinics, local transplant partners, community organizations, and other relevant parties. -Connects patients and their care partners with DaVita resources, community resources and social services to address social determinants of health, such as housing, food security, transportation, and financial assistance. -Provides telephone & In-person consultation and follow-up to support patients and their care partners. -Provides comprehensive support to potential living kidney recipients. By facilitating education about the living donor program, coordinating recipient's medical evaluations, and ensuring clear communication between the recipient, potential donor, and medical teams. This position will be open for a minimum of three days. The Wage Range for the role is $20.50 - $28.00 per hour. If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
