Position Overview We are seeking compassionate, highly organized, and patient-focused Health Management Coordinators to serve as the first point of contact for members joining our rapidly growing telehealth organization. This role is critical to creating an exceptional member experience by guiding patients through the onboarding process, gathering medical records and supporting documentation, coordinating telehealth appointments, and ensuring providers have all necessary information prior to patient consultations. The ideal candidate thrives in a fast-paced startup environment, enjoys helping patients navigate healthcare systems, and possesses exceptional communication and organizational skills. This individual will serve as a trusted resource for members throughout their onboarding journey while helping create a seamless virtual care experience.

Key Responsibilities

Member Onboarding & Engagement · Serve as the primary point of contact for new members entering the telehealth program. · Guide patients through the onboarding process from enrollment through their initial provider visit. · Educate members regarding telehealth services, program expectations, and next steps. · Build strong relationships with members and provide ongoing support throughout their care journey. · Maintain regular communication with members via phone, email, text, and patient portal platforms. Medical Record & Documentation Management · Collect and organize medical records, diagnostic reports, imaging studies, laboratory results, medication lists, and other clinical documentation. · Coordinate record requests with physician offices, hospitals, specialists, and healthcare facilities. · Verify receipt and completeness of required documentation prior to provider appointments. · Upload and organize patient records within the electronic medical record system. · Ensure providers receive complete and accurate patient information before telehealth consultations. Appointment Coordination · Schedule and confirm telehealth appointments. · Assist members with telehealth technology setup and troubleshooting. · Monitor appointment readiness and ensure patients complete all required forms and assessments. · Coordinate follow-up appointments and ongoing patient communications. · Track appointment completion and patient engagement metrics. Care Navigation Support · Assist patients in understanding care plans, next steps, and required documentation. · Facilitate communication between members and clinical teams. · Monitor member progress through onboarding and treatment pathways. · Escalate urgent concerns or barriers to appropriate clinical personnel. · Support retention efforts through proactive member engagement and relationship building. Startup Operations Support · Participate in process improvement initiatives designed to improve patient experience and operational efficiency. · Help develop scalable onboarding workflows and member engagement processes. · Assist leadership with special projects and operational growth initiatives. · Contribute ideas that improve patient satisfaction and organizational performance. Compliance & Data Integrity · Maintain HIPAA compliance and patient confidentiality at all times. · Ensure accurate documentation of all patient interactions. · Follow established procedures regarding medical records management and information security. · Maintain data integrity within patient management and electronic health record systems. Required Qualifications · High school diploma or equivalent required. · Minimum 2 years of experience in healthcare administration, patient services, medical office operations, care coordination, or telehealth support. · Experience obtaining and managing medical records and clinical documentation. · Strong customer service and patient engagement skills. · Excellent verbal and written communication abilities. · Proficiency with EMR systems, scheduling software, and Microsoft Office applications. · Ability to manage multiple priorities in a fast-paced environment. Preferred Qualifications · Associate's or Bachelor's degree in Healthcare Administration, Business, Public Health, or related field. · Experience working in telehealth, concierge medicine, chronic care management, or membership-based healthcare organizations. · Knowledge of medical terminology and healthcare workflows. · Experience with patient onboarding and care coordination programs. · Startup or high-growth healthcare company experience. · Familiarity with CRM platforms, patient engagement tools, and virtual care technologies.
