Job Summary

We are seeking an experienced Case Management Analyst to join our team on a part-time remote basis. As a Case Management Analyst, you will be responsible for assessing patients' needs, developing and implementing care plans, and coordinating with healthcare providers to ensure seamless care transitions. You will work closely with our clinical teams, patients, and families to promote optimal health outcomes and provide exceptional patient care.

Key Responsibilities

* Conduct comprehensive assessments of patients' needs and develop individualized care plans to address their medical, social, and emotional needs.
