About the Role

As part of our Chronic Care Management (CCM) outreach team, you’ll engage with eligible patients to book appointments with our nurses and GPs. Working primarily remotely, you’ll make outbound calls using our call centre systems, working with empathy, professionalism and a genuine desire to help improve patient health outcomes. We can entertain full-time and part-time opportunities!

What You’ll Be Doing

• Contacting eligible patients and booking them into CCM nurse and GP appointments
• Using patient lists and Practice Management Systems to verify eligibility and schedule appointments
• Ensuring medical centres are informed and supported throughout campaign activity
• Rescheduling or confirming appointments as required
• Recording call outcomes accurately and contributing to daily team check‑ins
• Collaborating with call centre colleagues and clinical teams to ensure successful patient engagement

What You’ll Bring

• Up to 2 years’ experience in a call centre, medical practice, or customer service role
• Outstanding phone manner and confidence in engaging with diverse patients
• Strong communication skills and a collaborative mindset
