Geriatric Home Health LVNAmcare Pro Home Health is a mid-size home health care company with offices in East Texas and Dallas. We are currently interviewing for a PRN LVN Field Nurse to cover the Tyler, Chandler, Flint, Bullard, Lindale and surrounding areas.We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high-quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.
Responsibilities:
- Provide skilled nursing care to home health patients under the supervision of an RN
- Perform routine nursing visits including vital signs, medication administration, injections, and treatments
- Assist with wound care and dressing changes per physician orders
- Monitor patient condition and promptly report changes to the supervising RN
- Educate patients and caregivers on medications, treatments, and safety
- Accurately document all visits and care provided in the agency EMR
- Ensure compliance with Medicare, Texas regulations, and agency policies
- Coordinate visits and communicate effectively with the clinical team
- Home health: 2 years (Required)
- Geriatrics: 1 year (Required)
- Spanish (Preferred)
- English (Required)
- LVN (Required)
- CPR Certification (Required)
- IV Therapy Certification (Preferred)
- Driver License
