Title: Home Plus Case Manager Location: New York United States Job Description: About The Role The Home+ Case Manager plays a vital role in providing trauma-informed, survivor-centered case management for individuals impacted by IPV and GBV. This position supports survivors in navigating complex systems including housing, financial resources, legal services, and safety planning to empower them to remain safely housed or transition to stable housing. We are seeking a passionate advocate who is deeply committed to social justice, anti-oppressive practices, and healing-centered engagement. The ideal candidate will approach their work through a racial, economic, and gender justice lens, recognizing the structural barriers that impact survivors' ability to access safety and stability. The position will report to our Rising Ground hub in Harlem, Manhattan. This position may require candidate to occasionally work from Manhattan Family Justice Center.

What You'll Do

Provide compassionate, survivor-centered case management to a caseload of 25-30 clients at any given time and serve at least 100 clients annually. Conduct comprehensive intake assessments, identifying each client's safety, housing, and financial needs. Develop individualized service plans rooted in client autonomy, self-determination, and harm reduction. Support survivors in navigating housing-related challenges, including public assistance applications, relocation planning, and financial aid. Provide crisis intervention, emotional support, and safety planning, ensuring services are trauma-informed and culturally responsive. Advocate for survivors in interactions with landlords, public assistance programs, healthcare systems, and legal services. Facilitate access to flexible funding for housing stabilization, emergency needs, and economic empowerment. Assist survivors in accessing the Office of Victims' Services (OVS) compensation program and other financial assistance resources. Connect clients to community-based organizations for legal advocacy, financial literacy, employment support, and mental health services. Develop and maintain strong referral pathways to ensure clients have access to a broad range of housing, legal, and economic support services. Engage in community outreach to increase awareness of Home+ services, especially among underserved survivors, including BIPOC, LGBTQ+, immigrant, and justice-involved individuals. Advocate for systemic change by addressing structural barriers that disproportionately impact survivors from marginalized communities. Work collaboratively with city agencies, grassroots organizations, and social justice groups to improve access to survivor-centered resources. Maintain accurate and confidential client records in compliance with HIPAA, VAWA, and other relevant privacy laws. Document client interactions, financial assistance allocations, and service outcomes in the program's case management system. Regularly assess program impact and participate in continuous quality improvement efforts to enhance service delivery. Work closely with the Home+ Coordinator and Program Supervisor to ensure that survivors receive holistic and culturally relevant support. Participate in weekly supervision, case conferences, and training sessions focused on trauma-informed care, restorative justice, and anti-oppressive practice.

Qualifications

Bachelor's degree in social work, psychology, human services, or related field preferred (Master's degree a plus). At least two (2) years of experience providing case management, advocacy, or direct services to survivors of trauma, violence, or systemic oppression. Deep understanding of the intersections of domestic violence, gender-based violence, housing instability, economic justice, and systemic oppression. Experience working with diverse populations, including BIPOC, LGBTQ+, immigrant, and justice-involved communities. Strong knowledge of NYC housing systems, public benefits, emergency financial assistance programs, and legal advocacy resources. Commitment to anti-oppressive, harm reduction, and restorative justice approaches. Ability to work independently and collaboratively within a team and across disciplines. Excellent organizational, communication, and crisis intervention skills. Bilingual (Spanish, Mandarin, Haitian Creole, or another language) preferred. Supervisory Responsibility No Work Environment: Hybrid (in-office and remote) with flexibility to meet clients in community settings or conduct virtual support sessions. Ability to travel within the assigned borough to engage with survivors and community partners. Position Type/Expected Hours of Work: Full-time position (35 hours per week), Monday-Friday, 9 AM-5 PM. Flexibility to work evenings and weekends as needed to accommodate survivors' schedules. Additional Requirements: Authorized to work in the U.S. Ability to work in-person in Harlem, Manhattan, NYC for at least 3 days or more per week. Ability to travel to other Rising Ground sites (if required)
