The Health and Wellness Coach facilitates telephonic lifestyle coaching sessions to create and maintain a culture of healthy habits and wellness initiatives in our members’ lives. They develop collaborative coaching relationships with their assigned participants, providing support, encouragement and education through program facilitation and motivational interviewing. What you’ll do

• Assess participant health, environment, nutrition and behaviors using a variety of assessments; identify specific goal(s) to support participant health and behavior changes.
• Utilize evidence based coaching skills to set health-related goals and develop/implement strategies to reach those goals.
• Provide coaching and education related to nutrition, exercise, sleep, stress management, tobacco cessation and other wellness topics.
• Learn and leverage motivational interviewing techniques when engaging with members virtually/telephonically.
• Provide exceptional customer service to members, providers, office staff, and other representatives via inbound and outbound calls and via online inquiries regarding claims, benefits, eligibilities, provider searches and other health-related topics.
• Complete outbound calls to engage with participants that are eligible for program enrollment and will provide necessary health coaching to reduce or eliminate high-risk behaviors, including Tobacco Cessation.
• Guide members and providers through the cost and complexity of healthcare; conduct research on members' behalf to resolve healthcare issues.
• Assure all member referrals are completed in a timely manner and documented according to client procedure.
• Use Quantum Health's systems and technology to accurately document all interactions related to patient care.
• All other duties as assigned.

What you’ll need

• Education: Bachelor's degree in a health-related field (e.g. Exercise Physiology, Public Health, Health Science, Nutrition, etc.) required.
• Strong working knowledge of current health issues and behavior-change strategies.
• Able to achieve and maintain role-related certifications (e.g. health education, Certified Health Education Specialist, health coaching, nutrition).
• Demonstrated ability to work with diverse populations and cultural backgrounds; Bilingual (Spanish/English) is preferred, not required.
• Strong customer service, active listening and interpersonal communication skills to identify critical core competencies based on success factors and organizational environment.
• Willing to accept flexibility in weekday working hours; needs to be available from 7:00 AM -10:00 PM based on business need
• Tobacco-free status for at least one year.
• Strong administrative/technical skills; Comfort working on a PC using Microsoft Office (Outlook, Word, Excel, PowerPoint), IM/video conferencing (Teams & Zoom), and telephones efficiently.
• Excellent verbal and written communication skills. Comfortable communicating with members and providers via phone regularly throughout the day.
• Trustworthy and accountable behavior, capable of viewing and maintaining confidential personal information daily.
