About the Role

Our Engagement Outreach Hub is a cutting-edge initiative focused on maximizing Medicaid member interaction through proactive outreach and engagement. As a Care Management Associate, you'll play a vital role in connecting with members, identifying their needs, and coordinating comprehensive care services. This is an exciting opportunity to join a dynamic team of healthcare professionals who share a passion for delivering high-quality care and support.

Key Responsibilities

  • Conduct telephonic outreach to Medicaid members, engaging them in care management services and promoting effective utilization of healthcare resources.
  • Facilitate case assignments, connecting members to care managers through warm transfers and ensuring seamless care coordination.
  • Provide exceptional customer service, addressing member concerns, and resolving issues in a timely and professional manner.
  • Maintain accurate and complete documentation of member interactions, adhering to regulatory requirements and company policies.
  • Collaborate with internal and external stakeholders, including care managers, healthcare providers, and community resources, to ensure comprehensive care coordination.
  • Adhere to compliance policies and procedures, maintaining confidentiality and protecting member information.
  • Meet daily metrics and productivity standards, demonstrating a positive attitude and enthusiasm for the role.

Requirements

To succeed in this role, you'll need:

  • Effective communication, telephonic, and organizational skills, with the ability to manage multiple priorities and adapt to change.
  • Strong customer service skills, with attention to detail, sensitivity to issues, and proactive problem-solving.
  • Ability to work independently and competently, meeting deliverables and deadlines while demonstrating an outgoing, enthusiastic, and caring presence.
  • Proficiency in electronic documentation, with thorough and accurate completion of member records.
  • Experience working in a team environment, including internal and external participants.

Preferred Qualifications

We'd love to see:

  • 2-4 years of experience in the healthcare field, preferably in a medical office, hospital setting, or medical billing/coding.
  • Proficiency in Microsoft Office, including Word, Outlook, and Excel, with data entry and documentation experience.
  • Familiarity with basic medical terminology and concepts used in care management.
  • Flexibility to work occasional nights and weekends outside of standard business hours (8:00 am - 8:00 pm).
  • Strong organizational skills, including effective verbal and written communication.
  • Bilingual (Spanish) skills.

Education

A High School diploma, G.E.D., or equivalent is required for this role.
